Information For Patients  

  • Please arrive 30 minutes prior to your scheduled appointment
  • Limited complimentary parking is available.Spots fill up quickly, so bring change for pay parking. 
  • Please bring any prior records and diagnostic studies (such as x-ray, MRI, CT-scan) with reports relating to your condition.
  • Medicaid referrals should be made out to Tulane orthopedics and are required at the time of your appointment booking.
  • School insurance forms must be filled out completely and signed by a school official in order to be seen.
